Preparing for Discharge

When your doctor feels that you are ready to leave the hospital he or she will authorize a hospital discharge.

When you are preparing to leave the hospital, a nurse, case manager, social worker, or discharge planner will help plan your follow-up care. Based on your condition, the hospital recommends patients be picked up by 10:00 AM.

The staff will discuss your discharge instructions with you and answer any questions that you may have. A staff member will also assist you from the unit to your transportation. Patients should check their room, bathroom and bedside table are fully for any personal items. And make sure they or their caregiver has all necessary paperwork for billing, referrals, prescriptions, etc.
